DO NOT SCHEDULE an appointment UNTIL you have VERIFIED all your information is CORRECT. Remember, just because you receive a letter does not mean your information is correct—VERIFY. You can return to schedule your appointment by returning to the website home page and click on “Member Login.” If you have difficulty verifying your information, please call us at 303-442-1275 and we will gladly help verify the information while you are on the phone.
